PARTICIPANT MAILING AND FORM 1095B
The Fund Office is conducting a mailing campaign to collect Social Security numbers and verify dates of birth for any dependents currently on the Health and Welfare Plan. Please refer to the H&W tab, under Participant Mailings for a sample of this letter.
Why Is This Being Done?
Social Security numbers are needed in order to provide Participants with Form 1095B. Without this information, the Fund will be unable to properly provide Form 1095B.
What Is A Form 1095B?
It is a statement which shows the months in 2015 in which a Participant had health care coverage.
Why Is Form 1095B Needed?
As part of the Affordable Care Act, in order to avoid paying a penalty, you will need to prove that during the year you had health insurance that qualifies as "minimum essential coverage". Your Health and Welfare Plan qualifies as having "minimum essential coverage".
What If I Do Not Have Minimum Essential Coverage?
For any month in which you or any of your dependents didn't have minimum essential coverage, or don't qualify for an exemption, you will need to make an "Individual Shared Responsibility" payment with the tax return you file in 2015.

Who Will Provide Form 1095B?
The Fund Office will mail Form 1095B to you.
The Federal Government is requiring that the Fund provide you with Form 1095B
When Will I Receive Form 1095B?
Form 1095B will be mailed to you by January 31, 2016
